Step 1: Apply for Indian Registration

CHILDREN OR DEPENDENT ADULTS - Application for Registration on the Indian Register and for the Secure Certificate of Indian Status (SCIS) (for children 15 years of age or younger or dependent adults)
Step 2: Once you receive confirmation from the government saying that you are an Indian, you have to apply again with a different form to get your status card
Secure Certificate of Indian Status (SCIS): Application for already registered persons
Circumstance #2: You have never had a Status Card but the Gender Equity ruling gave you the right to have one (for Bill C-3 Applicants Only)
You must meet ALL three of the following conditions:

the applicant's grandmother lost her entitlement to registration as a Status Indian as a result of marrying a non-Indian;
-
one of the applicant's parents is/was entitled to be registered pursuant to subsection 6(2) of the Indian Act; AND
-
the applicant, or one of his/her siblings of the same entitled to be registered parent, was born on or after September 4, 1951.
If you meet all three conditions, you are eligible and have to complete the following forms and mail them together to INAC:

If an eligible Guarantor cannot be obtained using Form No. Inter 83-120E, applicants must complete and submit the following form:

These forms are intended for all Registered Indians who require a Secure Certificate of Indian Status (SCIS). Please read over and complete the form carefully, because failure to complete all the sections of the form and submit all necessary documentation will result in your application being refused and returned to you. A complete application package allows for improved service through more efficient and timely processing of applications.
